When my wife came home after her ileostomy surgery, she began to try on her clothes, only to find it all begins with the underwear. She has a very small torso and her regular panties would not work over her ostomy bag. Obviously, we needed to find something that would not only work but be comfortable as well.

What is Ostomy Underwear?

In the U.S., there are between 750,000 to 1 million people who have undergone ostomy surgery. Over 100,000 ostomy surgeries are done each year. Ostomates use special undergarments designed to protect the ostomy bag from just hanging out there.

Ostomy undergarments are specially designed underwear that are worn by people who have ostomies. Most ostomates wear a simple pull-on over-the-briefs underwear because they’re easier to manage, but they can be unsightly and difficult to wash.

Even though my wife and I knew she would have a stoma after her surgery, we had no idea what it would be like. Sure, we saw pictures and peole tried t explain it to us, but until you have your own sticking out from your belly or side you’ll never fully undrstand.

People who have stomas usually wear two pairs of underwear: regular underwear and ostomy underwear with a special pouch that holds the ileostomy, colostomy bag.

It’s possible to buy ostomy underwear in different colors and designs. However, there are some general rules that must be followed to select the best ostomy underwear for women. First, you should consider the color and pattern of the ostomy underwear. Of course, this will make a difference depending where you will be wearing them.

You should also choose a pair of underwear that fits well. They should fit comfortably around the ostomy pouch and not be too tight or loose. If they are too tight, the stool will not be able to drop and fill the bag. This forces the stool to push sideways and I can tell you from personal experience it makes a mess and is absolutley a pain.

3. Knit Mesh Surgical Pants

These are my wife’s hang around the house ostomy underwear. The are not the nicest looking  undergarment but they are comfortable. We are frugal folks and we like that these can be washed at least a half dozen times on delicate wash. 

Just like the undies provided at the hospital, these net mesh boy shorts can hold in place the ostomy bag. While these are called maternity briefs we find them perfect for her stoma protection. The best ostomy undergarments for men are designed with comfort in mind. In fact, men with ostomies may have a harder time finding the best ones because they often need to be designed specifically for men who have ostomies. These can be uncomfortable, especially if they don’t fit properly. While there are several options available, the key to finding the best one is to do your research.

These boxer shorts were not designed to be ostomy underwear yet they work pretty good. They are easy to pull on and can be pulled up high enough to cover most ostomy bags.
